Home and Community Care

Provides a range of health care and support services to those who have acute, chronic, palliative or rehabilitative health care needs to help them remain independent and in their own homes for as long as possible.

Assists those who are eligible, and no longer able to remain in their homes, to access assisted living and residential care.

Will work with the person and the family to develop a care plan and make the necessary referrals. Some services are income tested.

Assisted Living:
  • Provides housing, hospitality services and personalized services for adults who can live independently but require assistance with daily activities.
  • Residences range from high-rise apartment complex to private home.
  • Hospitality services consist of meals, housekeeping, laundry, social and recreational opportunities and a 24 hour response system.
Residential Care:
  • Provides 24 hour professional nursing care and supervision in a protective, supportive environment for people who have complex care needs and can no longer be cared for in their own homes.
  • Includes meal service, medication supervision, assistance with daily activities and a planned program of social and recreational activities.
