Home Nursing Care Program
Professional nursing services are provided in their home to any person:
- With chronic and / or palliative nursing care needs
- To prevent admissions to hospitals and /or chronic care facilities
- To enable persons to return to their home following an acute hospitalization
- To provide palliative care to persons who wish to stay at home.
Referrals for assessment to receive this service are received from any source.
