These programs are open to adults of all ages, 19+, who are living with one or multiple ongoing health conditions 

 

 Tool Kit for Active Living with Chronic Conditions, Chronic Pain or Diabetes 


This evidence-based program includes all the tools in the Chronic Conditions, Chronic Pain or Diabetes  Self-Management Program, packaged into a one-time mailing. It offers an alternative method for those who cannot or do not wish to attend a program in person or online. The Tool Kit will give you some ideas to try over time and become an expert self-manager.

The tool kit contents:

  •  Living a Healthy Life Book 
  •  Relaxation for Mind and Body CD (not in Diabetes Kit) 
  •  An exercise CD with accompanying booklet  
  •  A self-test to help decide how to use the materials 
  • Tips sheets on the most important selfmanagement tools

 

Tool Kit for Active Living with Chronic Conditons, Chronic Pain or Diabetes Plus Call program

Participants in this 6-week program receive the materials from the Tool Kit for Active Living with Chronic Conditions, Chronic Pain or Diabetes Program, and participate in a weekly 45-minute, small group conference call with Program Leaders and other participants. The Leaders introduce the materials, explain the self-test to participants, does an exercise with the group, and gets each person to make a weekly action plan.  This program is very similar to our in-person community based programs we offered prior to COVID-19.

 

Better Choices, Better Health 

This web-based, asynchronous, 6-week program uses content from the Chronic Conditions Self-Management Program and focuses on decision making, problem-solving, and action -planning. New lessons are presented each week, and sessions are highly participative through internal messaging and online discussion boards. There are no requirements that participants log in at the same time.
